You are eligible if you are an unpaid carer, caring for someone with an intellectual disability, a physical disability, a mental illness, a chronic physical illness, or someone who is frail-aged or has dementia or needs palliative care

and

Your local government council is any one of the following: Adelaide, Burnside, Campbelltown, Holdfast, Marion, Mitcham, Norwood/Payneham/St Peters, Onkaparinga, Prospect, Unley or Walkerville.

To register by phone, please FREECALL1800 052 222 during business hours or call your local Carer Support centre. To have a member of the Respite team call you instead, email registrations@carersupport.org.au.

b) Complete it and save it to your computer, then email it to us at the address indicated at the top of the form’s first page.
